Tile Higher Than Hardwood Transition

Simply tiling on top of a plywood subfloor is not an option this practice is frowned upon and is not permitted by the tcna or ttmac.

How can i make this transition. Here the large slate tiles run right up to the wood end cap which also serves as a threshold between the spaces.
